Preparing for Your Visit


Here are some tips to help you prepare for your visit to our radiology office:

Insurance

Be sure that you are aware of what your insurance plan requires, including referrals from your doctor, preauthorization or pre-certification numbers, if applicable. You should also know your deductible amount, and know if you are responsible for paying your deductible. Co-payments are collected at the time of your appointment. 

Paperwork

Visit our Patient Forms page to access all of the materials necessary that you can complete before you come to our office. You’ll save time and be able to organize all of your vital medical information.

Before Your Exam

Be sure to follow the preparation guidelines for your specific digital imaging test. These can be found on the reverse side of our Patient Information Form. Before any exam, you should notify the staff if you have diabetes, kidney disease, or if you are allergic to any contrast material.
